KỸ THUẬT SIÊU ÂM KHỚP GỐI
Rất nhiều bệnh lý khớp gối liên quan đến gân, mạch máu, thần kinh, khớp và các cấu trúc quanh khớp có thể chẩn đoán chính xác bằng siêu âm. Mặt khác kĩ thuật này cũng có những hạn chế nhất là trong chẩn đoán các bệnh lý liên quan đến dây chằng chéo và sụn chêm do vậy người làm siêu âm khớp cần có hiểu biết về bệnh học để khắc phục các hạn chế này . Siêu âm khớp gối được thực hiện với đầu dò phẳng tần số từ 5- 10Hz.
I. Mặt cắt dọc qua gân cơ tứ đầu đùi: bộc lộ gân cơ tứ đầu đùi và túi hoạt dịch trên xương bánh chè. Cơ tứ đầu đùi là khối cơ chính ở mặt trước xương đùi được tạo nên bởi 4 cơ : cơ thẳng đùi, cơ rộng trong, cơ rộng giữa và cơ rộng ngoài. Bốn cơ này đi xuống dưới và chập lại thành gân cơ tứ đầu đùi bám vào bờ trên và hai bên bánh chè tạo thành gân bánh chè rồi tiếp tục đi xuống dưới tạo thành dây chằng bánh chè bám vào lồi củ chầy. Động tác của cơ tứ đầu đùi là duỗi và gấp đùi.
Túi hoạt dịch trên xương bánh chè là túi hoạt dịch nằm giữa xương đùi và gân cơ tứ đầu đùi. Túi hoạt dịch này thông với ổ khớp nên được ứng dụng để phát hiện tràn dịch khớp gối. Bình thường độ dày của dịch trong túi hoạt dịch trên xương bánh chè dưới 3 mm.

Sonographic Differentiation of Complicated From Uncomplicated Appendicitis: Implications for Antibiotics‐First Therapy
Abstract
Objectives
To evaluate sonographic findings as indicators of complicated versus uncomplicated appendicitis in the setting of known appendicitis, a necessary distinction in deciding whether to proceed with antibiotic therapy or with appendectomy.
Methods
With Institutional Review Board approval and Health Insurance Portability and Accountability Act compliance, appendiceal sonograms of 119 patients with histopathologically proven appendicitis were retrospectively blindly reviewed to determine the presence or absence of the normally echogenic submucosal layer, the presence of mural hyperemia, periappendiceal fluid, appendicoliths, and hyperechoic periappendiceal fat and to determine the maximum outside diameter. Results were compared with the presence of complicated versus uncomplicated appendicitis on histopathologic examination and assessed by both univariate and mulitvariate logistic regression; confidence intervals (CIs) of proportions were assessed by the exact binomial test.
Results
Thirty-two (26.9%) of the 119 patients had complicated appendicitis, including 11 with gangrenous appendicitis without perforation and 21 with gangrenous appendicitis and perforation. Loss of the submucosal layer was the only independent significant indicator of complicated appendicitis in multivariate regression (P < .001) and provided sensitivity and specificity values of 100.0% (95% CI, 89.1%–100.0%) and 92.0% (95% CI, 84.1%–96.7%), respectively.
Conclusions
Loss of the normally echogenic submucosal layer was the most useful sonographic finding for discriminating complicated from uncomplicated appendicitis, being the only finding independently and significantly associated with complicated appendicitis and, additionally, providing both high sensitivity and high specificity. This information may help a physician decide whether to proceed with antibiotic therapy or with appendectomy when treating a patient with appendicitis.

TIÊU CHUẨN CHẨN ĐOÁN ĐÁI THÁO ĐƯỜNG - VAI TRÒ CỦA HbA1c
Xét nghiệm HbA1c, giúp đánh
giá mức glucose
trung bình trong máu trong 2-3 tháng qua. Trước đây, HbA1c không được áp dụng trong chẩn đoán Đái tháo đường mà chủ yếu được sử dụng để theo dõi sự kiểm soát
glucose ở những người điều trị đái tháo đường. Từ năm 2010, Hiệp hội Đái tháo đường Mỹ (ADA) đã
chấp nhận xét nghiệm HbA1c như là một trong các phương pháp xét nghiệm chẩn đoán và sàng lọc đái
tháo đường bên cạnh các xét nghiệm đường huyết. Chỉ có các xét nghiệm HbA1c được
thực hiện từ phòng xét nghiệm được chấp nhận (chuẩn hóa) mới được sử dụng
cho mục đích chẩn đoán hoặc sàng lọc bệnh Đái tháo đường.
CHẨN ĐOÁN ĐÁI
THÁO ĐƯỜNG
Theo ADA dựa vào một trong 4 tiêu
chuẩn sau:
1. HbA1c ≥ 6,5%
Xét nghiệm nên
được thực hiện tại phòng xét nghiệm sử dụng phương pháp chuẩn.
Hoặc:
2.
Đường huyết đói ≥ 126 mg/dl (7.0mmol/l).
Đường
huyết đói được xác định là đo ở thời điểm nhịn đói ít nhất là 8 giờ
Hoặc:
3.
Đường huyết 2 giờ ≥200mg/dl (11.1mmol/l) khi làm test
dung nạp Glucose.
Test nên được
thực hiện như mô tả của WHO, dùng dung dịch chứa 75 gam glucose.
Hoặc:
4.
Bệnh nhân có triệu chứng cổ điển của tăng đường huyết hay
tăng đường huyết trầm trọng kèm theo xét nghiệm đường huyết ngẫu nhiên ≥200mg/dl(11.1mmol/l).
(Nếu không có các triệu chứng tăng đường huyết rõ
ràng, thì tiêu chuẩn 1-3 nên được lập lại)
ƯU ĐIỂM CỦA XÉT
NGHIỆM HbA1c:
Thuận tiện là một lợi ích của
các xét nghiệm HbA1c. Bệnh nhân không cần phải nhịn đói hơn 8 giờ (như đối với
xét nghiệm glucose huyết tương lúc đói) hoặc phải lấy nhiều mẫu máu trong vài
giờ (như đối với test dung nạp Glucose - OGTT).
Các xét nghiệm HbA1c phản ánh
mức trung bình của glucose trong máu trong 2-3 tháng qua.
Các xét nghiệm HbA1c không bị
ảnh hưởng bởi một số điều kiện có thể ảnh hưởng đến kết quả xét nghiệm glucose
máu như: bệnh nhân bị bệnh, đau đớn, hoặc bị stress vào ngày xét nghiệm.
Một ưu điểm khác của xét nghiệm
HbA1c là mẫu máu ổn định và có thể được lưu trữ ở nhiệt độ phòng lâu hơn so với
mẫu máu để xét nghiệm glucose.
Hiện tại được sử dụng rộng
rãi để quản lý và hiệu chỉnh theo mục tiêu điều trị cho từng bệnh nhân.
Là xét nghiệm được chuẩn hóa
cao theo DCCT/UKPDS.
HẠN CHẾ CỦA XÉT
NGHIỆM HbA1c:
1- HbA1c không phải là
lựa chọn cho tất cả mọi người.
Xét nghiệm này không nên được
sử dụng để chẩn đoán bệnh đái tháo đường trong một số trường hợp nhất định ảnh
hưởng đến kết quả xét nghiệm HbA1c như:
ü Phụ nữ mang thai.
ü Những người có bệnh thận mãn
tính.
ü Bệnh gan: nghiện rượu,
xơ gan, v.v…
ü Thiếu năng lượng
cấp.
ü Tăng
Triglyceride huyết.
ü Nhiễm độc
chì; nghiện ma túy; sử dụng thuốc: salicylas…
ü Những người bị rối loạn về
máu như thiếu máu thiếu sắt, thiếu máu do thiếu vitamin B12, sốt rét và các biến thể hemoglobin
như thalassemia, bệnh hồng cầu hình liềm.
ü Xuất huyết nặng gần đây và
truyền máu gần đây cũng dẫn đến sự thay đổi trong HbA1c.
2- Phương pháp cũng như thiết bị chẩn đoán HbA1c chưa được chuẩn hóa ở
nhiều nơi trên thế giới
Đó là một trong lý do ADA không khuyến cáo việc
đưa xét nghiệm vào tiêu chuẩn chẩn đoán đái tháo đường cho đến đầu năm 2010.
Từ giá trị của HbA1c, ta có
thể tính ra nồng độ glucose máu trung bình (MBG) theo công thức sau: MBG = 33,3x(HbA1c) – 86
Do đó, nếu chỉ sai số 1%
trong kết quả xét nghiệm HbA1c dẫn đến sai số tương ứng giá trị glucose máu là
33,3 mg/dl.
3- Giá thành của xét nghiệm cao
4- Cuối cùng, nếu sử dụng
tiêu chuẩn HbA1c để chẩn đoán thì số lượng người được chẩn đoán Đái tháo đường
sẽ thấp hơn và tỉ lệ tiền Đái tháo đường sẽ tăng lên khi so với sử dụng xét nghiệm
đường huyết đói.

New Algorithms in Prenatal Diagnosis
INTRODUCTION
The approach in prenatal diagnosis has been revolutionized by advances in prenatal molecular diagnostics. New algorithms in prenatal diagnosis are evolving and becoming increasingly complicated (Figure 1). The goal is to maximize the prenatal information for pregnant women and the families to make choices for the next generations.
Down syndrome screening has been the focus in prenatal diagnosis for a long time. However, many other chromosomal and structural foetal abnormalities were diagnosed incidentally during the screening programme for Down syndrome, coupled with the 18–22 weeks foetal anomaly ultrasound. Combined screening at 11–13 weeks by maternal age, foetal nuchal translucency (NT) thickness by ultrasound, maternal serum pregnancy-associated plasma protein A (PAPP-A) and free beta-human chorionic gonadotropin (β-hCG) can identify 90% of foetuses with trisomy 21 (Down syndrome) and other common aneuploidies (trisomies 18 and 13) at a false-positive rate (FPR) of 5%.1
The performance is highly reproducible worldwide and has also been demonstrated in the Hospital Authority universal Down syndrome screening programme in Hong Kong from 2010.2 Additional ultrasound and maternal serum markers with different contingent screening tests have been studied to further improve the detection rates and to reduce the FPR.
An important breakthrough in prenatal screening using maternal plasma cell-free foetal DNA as a noninvasive prenatal testing (NIPT) for foetal chromosomal abnormalities was discovered by Professor Dennis Lo from Hong Kong.3 The detection rate for Down syndrome using NIPT is >99% with an FPR of as low as 0.1%. NIPT can be performed using maternal blood sample from 10 weeks’ gestation onwards. NIPT is currently available as a secondary screening tool for pregnancies with positive conventional Down screening as well as for primary screening for Down syndrome.4-6
For prenatal diagnosis in the 21st century, traditional karyotyping is no longer adequate. Advances in prenatal molecular diagnostics including polymerase chain reaction (PCR) as rapid aneuploidy test, and chromosomal microarray (CMA) as molecular karyotyping including the detection of microdeletions and microduplications, are going to replace traditional karyotyping sooner or later.7 There are also new modalities being developed such as whole exome sequencing, WES (sequencing all the protein-coding genes in the genome) or whole genome sequencing, WGS (sequencing the entire genome). WES or WGS will not be discussed in this review article.
Despite the rapid ongoing development in prenatal molecular diagnostics, ultrasound maintains a pivotal role in the new algorithms (Figure 1), being the link between the various tests inside the algorithms. In modern prenatal diagnosis, continued utility of ultrasound (with corresponding counselling) could offer career sustainability for foetal medicine specialists.
This review article is focused on the new algorithms in prenatal diagnosis (Figure 1) from a clinical service/clinical pathway point of view. The current situation in Hong Kong will be referred to from time to time for illustration. It is beyond the author’s capacity to go in-depth into the laboratory aspects.
